Accounting and grants management forms

• CS&E department funding request - Complete this form to request approval of department funds for special projects or requests.
• CS&E quick preaward intake form - Submit this form as soon as you know you are submitting a proposal.
• Faculty Chart of Accounts (COA) - Sign in with your UMN authentication to see a list of your account strings and current general ledger balances. Please confirm balances with your accountant before making any high dollar commitments.
• Faculty to accountant roster - You must be on VPN or connected to the University network for access.

Communications and CSE-IT forms

• Poster printing request form - Submit a poster printing request to CSE-IT.
• Submit your news & story ideas - Share your special projects, research news, and story ideas for inclusion in departmental communications.
• Website content change request form - Use this form to request any changes or updates to the CS&E website.

Human resources and payroll forms

• CS&E computer account request form - This form is for authorizing the use of various CS&E computing resources, including the grad machines.
• Request for employment verification letter (online) - Requires an x500 login.
• Visa request form - Please fill out this form to request a J-1 scholar or H1-B employment based visa.

Purchasing and travel forms

• Computer Science ePurch system (must be connected to VPN to access) - contact [email protected] with questions or concerns.
• Chrome River Delegation Intake Form (Faculty Use Only) - contact [email protected] with questions or concerns.
• CS&E FedEx Request Form - Please complete this form to request an outbound FedEx shipment.

Space and access forms

• CS&E access request form - Use for all key, U-Card swipe, and after-hours access for the Department of Computer Science & Engineering.
• CS&E graduate student lounge (Keller 2-212) access request form - Currently enrolled Computer Science and Data Science graduate students can use this form to request or update swipe card access to the grad lounge.
• Shepherd Labs access request form - Use for U-Card swipe and after-hour access to Shepherd Labs.
• CS&E computer account request form - This form is for authorizing the use of various CS&E computing resources, including the grad machines.
• Keller 4-201 lactation space code request - Submit this form to receive the code to access the dedicated lactation space in Keller Hall.
